Our initial consultation questionnaire has been updated and is now available in multiple formats.

We just uploaded a new consultation questionnaire to the downloads section. It's broken out better for easier use, has some new sections, and now it's available in Excel or PDF formats, so you can modify the sizes of text fields if you have Excel, or print it out from the PDF and fax it to us if you like!
